The Evanston CBOC sees patients by appointment only--this clinic is not an urgent care facility.

In case of emergencies, please call 911 or go to the nearest emergency room. The following resources may also be helpful:
  • The Nurse Advice Line is available 24/7 at 1-888-598-7793.
  • If you opt to use a non-VA Emergency Room, please notify the Emergency Room Notification line at 1-844-724-7842 within 72 hours. 
  • If you visit a non-VA urgent care facility, the urgent care facility is required to call 888-901-6609 to cover your visit. This interactive phone system is available 24/7.
  • If you are having suicidal thoughs, please call 988 and press 1 to be connected with the Veterans Crisis Line (VCL).
    • You may also text 838255 or chat online at veteranscrisisline.net/chat.
    • You don't have to be enrolled in VA benefits or health care to connect with the VCL. The VCL is:

Registration/Enrollment

Patients must be registered, enrolled and eligible to receive services in our clinic. The first step is to complete an Application for Health Care Benefits (VA Form 10-10EZ). Primary Care

All eligible Evanston CBOC patients are assigned a primary medical care provider. Together with the clinic staff, your primary care provider coordinates your care within the VA health system. Services offered through the Evanston Community Based Outpatient Clinic include:
  • Diagnosis and management of your health care needs
  • Health promotion and health wellness education
  • Special needs
  • Senior care
  • Laboratory services
  • Pharmacist-managed Coumadin Clinic (available on site two days per week)
  • Other services

Audiology Services

The Audiology Clinic at the Evanston CBOC provides comprehensive assessment and rehabilitation of patients with hearing and balance disorders.

To schedule an appointment with Audiology please call 224-610-3759.  Please be sure to tell the scheduler you would like to be seen at the Evanston CBOC. We also provide audiology services at our North Chicago location.

Audiologists administer diagnostic and rehabilitative audiology services, including audiometric testing and consultations, determine the need for hearing aids or other assistive communication devices, makes selections, conducts fitting of hearing aids/assistive listening devices, provides instruction on the use and maintenance, programs/reprograms and repairs as needed.

Audiology services provided include:

  • Hearing aid evaluation, fittings, programming and repairs
  • Cerumen management
  • Tinnitus management
  • Tele audiology
  • Benign paroxysmal positional vertigo (BPPV) screenings
Before scheduling an appointment for hearing aid problems have you tried:
  • Cleaning the earpiece and/or chainging the filter, if applicable?
  • Changing the batteries.
  • To repair your hearing aid, you can:
    • Utilize our secure hearing aid repair drop off boxe inside the Evanston CBOC.
      • This box is located inside the main doors at Evanston CBOC.
      • Place your aids in the envelope provided, and include the following information, and allow 3-4 weeks for the hearing aid to be repaired:
        • Date
        • Full Name
        • Last four of your Social Security Number
        • The issue(s) you are experiencing
        • How would you like it returned to you after repair (mail or pick- up)
    • Mail your aid(s) to

      • Once the hearing aids are repaired, they will be returned via mail (unless other arrangements are made to pick them up).  Please allow 3-4 weeks turn-around time.Women’s Health Services

Women's Health

We are committed to meeting the unique needs of Women Veterans by delivering the highest quality health care to each woman, while offering her the privacy, dignity, and the sensitivity she deserves. Comprehensive primary care is offered to Women Veterans at the Evanston Clinic. Services include well women exams, preventive health care and gynecological services.

Social Work

The Evanston CBOC offers community-based programs for:
  • Readjustment counseling
  • Social services support
  • Individual therapy
  • Family therapy
  • Family education & counseling
  • Relapse prevention
  • Acute/long-term inpatient treatment

Mental Health Services

Evanston CBOC offers mental health services to include psychiatric assessments, psychiatric medication management and neuropsychological testing when clinically indicated. In addition, Licensed Clinical Social Workers (LCSWs) provide individual, couples/family and group therapy. These services can be offered face to face or via video conferencing (VA Video Connect).

Mental Health treatment at the Evanston CBOC emphasizes the following:

Who is eligible for services at this location?

All veterans eligible for VA care. The Evanston CBOC does not serve active-duty service members.

What is the anticipated length of treatment?

The length of treatment varries depending on your needs.

How can I access these services?

Call us to schedule an appointment. You can also ask your primary care provider, or another at our other locations, to submit a Mental Health Consult on your behalf requesting outpatient psychiatry and/or individual psychotherapy at the Evanston CBOC.
 

Referrals

You may be referred to our North Chicago facility (our parent facility) for specialty consultations, such as Ophthalmology (eye exams), Cardiology (heart specialists), Pulmonary, Gastroenterology, etc.
