VHIE Veteran Health Information Exchange

What is the VHIE (Veteran Health Information Exchange)?

The VHIE is a system that allows your VA/FHCC provider and your participating community care provider to securely access certain parts of your health record. This access reduces the need for you and your families to request and carry paper medical records from one health care provider to another.

Sign a form so your FHCC and Non-FHCC healthcare providers can securely share your health records electronically.

Your doctor will have a better picture and more complete summary of your health by:

  • Easily sharing your current medications with your doctors
  • Eliminating unnecessary medical tests
  • Improving your health by empowering you

How do I sign up for the VHIE?

Complete VA Form 10-0485 and return it as outlined below.

When you visit our campus in North Chicago, you can present the form with photo ID to Bldg. 133 1st floor, MyHealtheVet Kiosk near Primary Care; or Bldg. 133 basement, Release of Information

At this time, the list of health care system partners with the Veteran Health Information Exchange is limited. The list of current participating providers in the area is available at the Release of Information Office or at the MyHealtheVet Kiosk.
